Total Care 2 U (TC2U) is a healthcare company providing residence-based, comprehensive primary care for chronic Medicare and "dual eligible" patients. These seniors are most often 75 years or older and on Medicare Part B. A physician, along with a hand selected nurse practitioner, leads a Total Care Team (TCT or comprehensive care team), who collectively take responsibility for the ongoing care of patients with chronic conditions. The local Total Care Team manages all of the patient's medical needs and takes responsibility for arranging necessary care with other qualified professionals. Additional health care services may include care for all stages of life; acute care; chronic care; preventative services and hospitality care, and coordinating all products and services to achieve the highest possible outcomes at the lowest possible cost. At the doctor's recommendation, TC2U identifies at-risk patients, implements standardized treatment protocols to minimize complications, improves outcomes, and reduces the expense of emergency room visits and unnecessary hospitalizations or skilled nursing facility admissions. TCT's develop and grow a bond with their patients to better provide continuous and comprehensive senior care. Care is coordinated and integrated across all elements of the complex healthcare system (e.g. subspecialty care, hospitals, home health agencies, nursing homes, CCRCs, assisted living facilities, hospitalists, independent living) and the patient's community (e.g. family, public, and private community-based services).
