Job Summary

The APP will work in the Post-Transplant Cardiology Clinic and provide comprehensive care. The APP will work closely with the attending physician and other members of the healthcare team to manage the care of post cardiac transplant patient population. Schedule is M-F from 8-4:30PM.

Responsibilities:

  • Provide comprehensive post-transplant care based upon program Clinical Practice Guidelines (CPG's).
  • Order and interpret diagnostic tests
  • Prescribe medications and other treatments as appropriate
  • Collaborate with the healthcare team
  • Educate patients and their families about their condition and treatment options
  • Maintain accurate and complete documentation of patient care in the medical record
  • Participate in policy, protocol, and guideline development and revision
  • Participate in education, quality initiatives and research project

Qualifications:

  • Acute Care Adult Gerontology NP, Acute Care NP, or a Physician Assistant
  • Transplant experience preferred
  • Excellent communication and leadership skills
  • Ability to work effectively in a team and independently
  • Commitment to ongoing professional development and process improvement
